Identity Verification

The Department of Education selects some students for identity verification, with specific procedures for the student and school to follow.

Students are not allowed to complete this verification process online through electronic forms. There are two options to satisfy these verification requirements:

• Visit a PRCC campus in person
  • Show your ID in person at any PRCC campus.
    • Poplarville – Office of Financial Aid (Administration Building)
    • Forrest County Campus – Student Services (Building 3)
    • Hancock Center
  • Bring your unexpired government-issued identification so that we can make a front/back copy.
• Complete the identity verification form with a notary if you are not able to visit a PRCC campus location
  • Request the form by e-mailing finaid@prcc.edu.
  • Bring the blank form to a notary so they can witness your valid identification document.
    • PLEASE NOTE: The notary MUST be in person. You are NOT allowed to use an online notary service or perform this remotely via video or web access.
  • Have the notary complete their part of the form.
  • Make a front/back copy of your unexpired government-issued identification.
  • Mail the completed form and copy of your ID to the address listed below.
  • IMPORTANT: we cannot accept e-mailed or faxed copies of these documents. You MUST mail the originals to our office.
