Identity Verification & Statement of Educational Purpose
The Department of Education selects some students for identity verification, who then need to sign a statement of educational purpose.
Students are not allowed to complete this verification process online through electronic forms. There are two options to satisfy these verification requirements:
- Visit a PRCC campus in person
- Sign the form in person at any PRCC campus.
- Poplarville – Office of Financial Aid (Administration Building)
- Forrest County Campus – Student Services (Building 3)
- Hancock Center
- Bring your unexpired government-issued identification so that we can make a front/back copy.
- Sign the form in person at any PRCC campus.
- Complete the Statement of Educational Purpose form with a notary if you are not able to visit a PRCC campus location
- Request the form by e-mailing finaid@prcc.edu.
- Bring the blank form to a notary so they can witness your signature.
- PLEASE NOTE: The notary MUST be in person. You are NOT allowed to use an online notary service or perform this remotely via video or web access.
- Have the notary complete their part of the form.
- Make a front/back copy of your unexpired government-issued identification.
- Mail the completed form and copy of your ID to the address listed below.
- IMPORTANT: we cannot accept e-mailed or faxed copies of these documents. You MUST mail the originals to our office.
